The in-camera application for 'Witness S' was filed on Thursday.
The officer had conducted covert operations to gather intel about an alleged organised crime syndicate and its infiltration into the South African Police Service (SAPS).
The records have been briefly declassified for the commission only.
Evidence leaders warn that revealing this publicly may alert suspects they're being tracked or may lead them towards informants.
In-camera hearings are set to begin on Monday.
